STATEMENT OF NON-DISCRIMINATION

Faith Christian School reserves the right to select students on the basis of academic performance and personal qualifications.  FCS does not discriminate on the basis of race, color, national, or ethnic origin in administration of its educational policies, admission policies, and athletic or other school-administered programs.  Attendance is a privilege.  This privilege may be forfeited by those who do not conform to the standards and regulations of the school.

PRESCHOOL APPLICATION AND ADMISSION PROCEDURES

Preschool application

Afternoon Enrichment Program Fact Sheet (3-5 year olds)

Step 1. Evaluate your child’s birthdate. Three year olds and four year olds must reach these ages before September 1st in order to enroll in preschool classes. No exceptions.

Step 2. Be sure your child is potty-trained.

Step 3. Complete all required forms:

  1. Enrollment Form
  2. Emergency Health Card (front & back) – You must have 3 local emergency contacts listed on your card before your child is considered enrolled.  No exceptions!
  3. Pick-up List
  4. Statement of Cooperation (signed by both parents)
  5. Enrollment Agreement
  6. Parental Permission slip regarding “Accidents”
  7. Release of Personal Identifiable Information Form
  8. PEP Volunteer Survey
  9. Certificate of Immunization Status Form
  10. Copy of custody papers if parents are divorced.

Step 4. Pay application fee of $30.00 and registration fee.
These fees are non-refundable.

KINDERGARTEN APPLICATION AND ADMISSIONS PROCEDURES

Kindergarten application

Step 1.  Evaluate your child’s birthdate. ~Five year olds must reach this age before Sept 1st in order to enroll in Kindergarten classes. No exceptions.

Step 2.  Fill out all required forms.

  1. Enrollment Form
  2. Emergency Health Card (front & back)- You must have 3 local emergency contacts listed on your child’s card before your child will be considered enrolled.  No exceptions!
  3. Pick-up List
  4. Statement of Cooperation (signed by both parents)
  5. Enrollment Agreement
  6. Release of Personal Identifiable Information Form
  7. PEP Volunteer Survey
  8. Certificate of Immunization Status Form
  9. Parental Permission slip regarding “Accidents”
  10. Copy of custody papers if parents are divorced.

 Step 3.   Make appointment for student testing and family interview.

a.  Each incoming student must be tested for grade placement.
b.  A personal family interview is required for new students.  An appointment for this interview will      be scheduled after the completed application is received.

Step 4.   Pay application fee of $30.00 and registration fee. These fees are non-refundable.

GRADES 1ST – 4TH APPLICATION AND ADMISSION PROCEDURES

1st grade through 4th grade application

Step 1.   Complete and return all attached forms to the school office.

  1. Application with copy of your child’s last report card
  2. Records Release
  3. Emergency Health Card front & back.Note: three (3) local emergency contacts other than
    parents must be listed.
  4. Certificate of Immunization Status Form
  5. Release of Personal Identifiable Information Form
  6. Pick-up list
  7. Statement of Cooperation (one per family)
  8. Parent’s Code (one per family)
  9. For Grades 4th-8th only: Student Commitment Form
  10. Parents as Educational Partners form (PEP Volunteer Form) (one per family)

Step 2.   Pay Application fee of $30.00. Non-refundable fee covers the initial processing expenses.  (Note: completed application is not a guarantee of admission, only a request for admission.)

 Step 3.  Make an appointment for student testing and family interview.
